– Titrate individually in 100 mg increments every 5 days to an effective dose that minimizes adverse reactions
-Maximum dose: 300 mg / day

For patients CURRENTLY receiving immediate-release (IR) tramadol:
Starting Dose: Calculate 24-hour IR requirement and start with a total daily dose of ER rounded down to the next smaller increment of 100 mg orally once daily

Conversion from other opioids:

Discontinue all other opioid medications for the whole day before starting therapy
– Starting dose: 100 mg ER PO once daily
– Titrate individually in 100 mg increments every 5 days to an effective dose that minimizes adverse reactions
-Maximum dose: 300 mg / day.
Usual geriatric dose for pain:
